PO-02-044 HIS CARDIAC RESYNCHRONIZATION THERAPY: A NETWORK META-ANALYSIS

چکیده

Cardiac conduction system pacing, in the form of His-Cardiac Resynchronization therapy (His-CRT), is advantageous due to its physiological nature and accompanying clinical benefits.His-CRT has not become mainstream practice, challenges such as higher procedural time, pacing thresholds varying success rates. There a paucity studies comparing His-CRT with other modalities Left Bundle Branch Paced-Cardiac Therapy (LBBP-CRT) or Bi-Ventricular-Cardiac Therapy(BiV-CRT). To investigate electrophysiologic outcomes patients receiving compared LBBP-CRT BiV-CRT. We performed systematic search PubMed, EMBASE, Scopus databases identify BiV-CRT for appropriate indications. Eight were included final analysis. The studied differences ejection fraction (EF), QRS duration, capture threshold ventricular impedance at baseline follow-up. A frequentist network meta-analysis model was used. used netmeta module random effects CRAN-R software compute pooled standard mean across all outcomes. Higgin’s I2 statistic evaluate heterogeneity. Results are reported using reference treatment. Pooled Standard Mean Difference (SMD) follow-up EF comparison - 5.66% (95% CI: -10.6 0.73; p-value = 0.024) while -0.87% -5.53 3.79; 0.714). Prospective trials needed further substantiate our findings.

Cardiac resynchronization therapy: a meta-analysis of randomized controlled trials.

BACKGROUND Studies of cardiac resynchronization therapy in addition to an implantable cardioverter defibrillator in patients with mild to moderate congestive heart failure had not been shown to reduce mortality until the recent RAFT trial (Resynchronization/Defibrillation for Ambulatory Heart Failure Trial). We performed a meta-analysis including the RAFT trial to determine the effect of cardia...

Cardiac Resynchronization Therapy and QRS Duration: Systematic Review, Meta-analysis, and Meta-regression

Cardiac resynchronization therapy (CRT) has been shown to reduce the risk of death and hospitalization in patients with advanced heart failure with left ventricular dysfunction. However, controversy remains regarding who would most benefit from CRT. We performed a meta-analysis, and meta-regression in an attempt to identify factors that determine the outcome after CRT. A total of 23 trials comp...

Cardiac-resynchronization therapy.

Cardiac resynchronization therapy (CRT) addresses abnormal left ventricular (LV) activation that produces detrimental effects on cardiac systolic and diastolic function. CRT improves symptoms and ventricular performance, promotes reverse remodeling, and decreases mortality and hospitalization in patients with congestive heart failure (CHF). Atrial-synchronized biventricular stimulation reverses...

Cardiac Resynchronization Therapy

According to the 2013 update on heart disease statistics from American Heart Association, an estimated 5.1 million Americans ≥20 years of age have a diagnosis of heart failure. In the year 2009, heart failure was noted to be the primary cause of death in >56 000 people and contributed to death of >274 000 people. It is one of the most common causes of hospitalization in patients ≥65 years of ag...

[Cardiac resynchronization therapy].

Fifteen years after the birth of resynchronization therapy, this technique has become one of the recommended treatments in severe heart failure patients, with low ejection fraction (4 35%), and conduction disorders (ORS > or = 120 ms). Based on large randomized clinical trials, the European recommendation is class I type (level of evidence A), to improve symptoms and survival of this patient po...
